Processes relating to community outreach activities

Prepare a simple diary that your volunteers or staff can complete during outreach activities. It provides a snapshot of what happened during each activity. The data captured is useful for reporting on how you have engaged with expectant parents and the type of support provided.

Please download below an example booklet with six diary sheets for peer supporters to complete. The data captured can be transferred onto an excel sheet.
